Please find below a list of all necessary documents for participating in the Olympiad.
Document Submission Instructions

Stage I includes documents completed by parents or the participants themselves, which are required to finalize registration.
Stage II includes medical documents and the notarized power of attorney, which are subject to later upload as they must be prepared prior to arrival at the Educational Center.
All participants, team leaders and observers must upload their documents independently through their individual registration forms.

6**. Consent of the legal representative (parent) for medical treatment — required for children under 15 years of age.
II stage
7**. Consent form assigning responsibility for the underage student’s health and safety to the Team Leader.
8. Medical Clearance Certificate for Sports Activities. Students who are 15 years or older may complete this template independently.
9. Certificate of absence of contact with patients with infectious diseases within the last 21 calendar days.

Frequently Asked Questions
- Q:Regarding a child’s name, I noticed that the form requests the following order: Surname, First Name, and Patronymic. To maintain consistency with official documents, would it be acceptable to write the name exactly as it appears in the passport?A:Certainly. Please use the name order exactly as it appears in the passport.
- Q:Could you please confirm whether a signature from a family doctor is sufficient for Forms 9 and 10, or if these forms require a signature from a specific medical specialist or authority?A:For Forms No.9 and No.10, a family doctor’s signature is acceptable.
- Q:The Form No.10 states that the individual must not have been exposed to an infectious disease within the past 21 calendar days. For our submission, does this refer to the 21 days leading up to and including November 6?A:The ‘past 21 calendar days' refers to the 21 days before the submission date or a key deadline. If the due date is November 6, the 21-day period would be from October 16 to November 6.
NB: The certificate will only be valid if issued after October 26. - Q:Is it necessary for Form No.8 to be stamped by a notary office, or is it enough to have only the parent’s signature? We ask because local notary offices typically only provide certification for documents in our native language.A:A parent or guardian’s signature is enough; notarization is not required. One or both parents' signatures are acceptable.
